of Interest approved to move on to the Grant
Application stage, your proposal must include
the following information:
Completed
Grant Application Form
List of your
Board of Directors, including names, titles
and affiliations
Copy of your
organization's IRS 501 (c) (3) Determination Letter
Applicant’s
Personal Faith Statement
Current annual
operating budget detailing expenses and
anticipated revenue sources
Year to date Profit and Loss Sheet
Detailed project or program budget listing expenses and anticipated revenue sources (three year budget for start-up)
Current Balance Sheet
Business Plan for the enterprising component
Copy of your organization's most recent audited financial statements
Letters of
support or references (maximum of three)
